To give a membership in LCCA to someone, fill-in and submit this online gift membership application or download and mail the printed version of the form.

PLEASE NOTE:

A separate form must be submitted for each individual receiving a gift membership.
Incomplete forms or incorrect fees will cause the application to be returned.

New members pay a one-time-only Initiation Fee of $10.

Membership dues are based on location.

Recipient Member (Inside Cont'l US)	$40.00	$30 annual dues, plus $10 Initiation Fee (one time only)
Recipient Member (Outside Cont'l US)	$52.00	$30 annual dues, plus $10 Initiation Fee (one time only), plus $12 postage surcharge (AK, HI, & foreign)

The dues payable with this application are for one year. Dues for subsequent years will be billed on the quarterly date closest to the anniversary date of the application.
